Adoptions
All adoptions filed in Clark County must be finalized by the Probate Court. Those wishing to adopt are required to submit to a
personal home investigation to insure their suitability as adoptive parents. The hearings and records are confidential by law.
| Release of Adoption Information |
To obtain copies of a Final Decree & Adoption Certificate issued by this Court you must:
- Be the Petitioner or Adopted Person
- Come to the Probate Court and Show your ID or proof of identity
- Pay $5.00
Beginning March 20, 2015 legislation was passed in the Ohio Senate stating, all other information
will be released through the Ohio Department of Health, Office of Vital Statistics, rather than through county probate
courts. Adoptees will be able to complete an Application for Adoption File and Authorization for Release of Adopted name,
which forms will be available on the Ohio Department of Health website:
